Payment can be made by cheque when you send in the forms below, but it can also be made online with credit card or debit card through PayPal's secure system. You do not need to sign up for a PayPal account, as there is a small link to skip that step.

Proctor agreement - to be filled out by the person supervising the exam
Affidavit - important agreement to be signed after passing the exam
Application for certification - when you have completed your 2 years experience and all other requirements
Application for certification renewal - sent when it is time to renew
